Quality Behavioral Health, a progressive outpatient psychiatric and psychotherapeutic Group in Warwick, RI is seeking to add Licensed Social Workers to support our new Integrated Medical and Lifestyle program. The model, along with the recent introduction of a Pediatric Program, has resulted in a dramatic increase in patient referrals, and the need to add supporting staff. We are hoping to draw upon the skills and experience of Licensed Independent Clinical Social Workers to provide both Care Coordination and Counseling to the large and growing population of children, adults and families looking for relief from the pressures of daily living, to help ease the pain of mental health symptoms, and to provide guidance and access to resources that support the many social factors that determine the quality of health and wellness.
The Care Coordinator will work closely with members of our Health and Wellness teams: Board Certified psychiatrists, psychiatric nurse practitioners, LICSW’s, and LMHC’s, and a Registered Nutritionist, determining patient needs, developing care plans, and coordinating patient care services with members of the team and with other healthcare providers and specialists. You will be in regular contact with patients, and collaborating medical and resource providers to ensure adherence to the plan and to ongoing inter-provider communication.
The successful candidate will display a compassionate desire to help individuals and their families gain access to supports available in their homes, neighborhoods, communities and to outside specialists. You will emphasize to all in your care the effect of social determinants on their health. Housing concerns, income, making healthy food choices, exercise, and access to community health supports have been shown to have a significant impact on general health and wellness outcomes, especially with patients exhibiting behavioral health symptoms.
Required is LICSW certification and 2 years of related experience.
